Job Description:
The Early Childhood Coordinator plays a pivotal role in providing exceptional care and support to children with disabilities, their families, and individuals with disabilities.
* Conduct assessments for children aged 0-9 years, utilizing expertise and consulting with Allied Health Professionals when necessary.
* Deliver targeted interventions using evidence-based models and collaborate with professionals as required.
* Support access to the NDIS when applicable and develop, implement, and review personalized plans.
Key responsibilities include facilitating individual or group sessions, cultivating relationships with community groups, organizations, government services, and service providers, and linking individuals to relevant resources.
The successful candidate will work closely with families to empower them, building capacity and confidence to live fulfilling lives by setting goals, exercising choice, and accessing mainstream supports.
