Key Responsibilities
- Manage a client case‑load planning and review client goals to ensure high-quality, person‑centred care.
- Communicate proactively with clients, families, and team members to develop and update care plans.
- Meet with clients and families face‑to‑face to review care plans and ensure needs are being met.
- Accurately document all client interactions and share relevant information with the team.
About you
- Previous and relevant experience working with seniors or aged care.
- Background in care coordination, case management, or client‑facing support roles.
- Experience building relationships with clients and families, preferably in a home care or community setting.
- Able to work under pressure and multitask shifting priorities.
- Desirable: qualification in a health‑related field such as nursing, allied health, social work, or community services.
- Administrative experience and competent in using computers and systems.
