The GP Assistant will support the practice team by accurately summarising patient medical records and scanning all relevant documents into the clinical system, ensuring records are complete, accurate, and coded in line with NHS standards.
This role helps maintain high-quality patient data and frees up GP time to focus on clinical care by working closely withpractice teams and the wider PCN to ensure services aredelivered efficiently and experiences are improved.

Main duties of the job
Read and interpret incoming patient records (e.g., paper notes, hospital letters, discharge summaries).
Accurately summarise medical histories into the clinical system (EMIS/SystmOne) using appropriate Read Codes or SNOMED codes.
Ensure all relevant clinical information is captured and recorded in the correct format.
Scan paper documents into the clinical system, attaching them to the correct patient record.
Apply accurate coding and filing to ensure easy retrieval and compliance with data quality standards.
Check that all scanned documents are legible and filed in the correct section of the patient record.

Job responsibilities

Review all incoming correspondence filing the document and entering pertinent information into the patients electronic medical record in accordance with the Coding Protocol with attention to QOF.
Highlight clinician action and workflow to appropriate clinician or team as per Workflow Protocol.
Organise the medical record of patient whom you are coding, to link problem titles.
Checking incoming medical records and summarising onto clinical system.
Recognise own knowledge limits and seek advice from clinical or administrative colleagues as appropriate.
To carry out administrative related tasks as delegated by Clinical Lead and Practice Manager
Maintain patient confidentiality and comply with GDPR and NHS Information Governance requirements.
Undertake relevant training to maintain knowledge and skills.
Follow all practice and PCN policies and procedures.

Person Specification
Qualifications
Good standard of education (GCSE or equivalent).
Previous administrative or records management experience in a healthcare setting.
Strong IT and typing skills; attention to detail; understanding of confidentiality and data protection.
Organised, reliable, able to work to deadlines, good communication skills, team player.
Training in medical terminology or health administration.
NHS primary care experience; previous summarising & scanning work.
Knowledge of EMIS or SystmOne; coding with SNOMED or Read Codes.
Flexible approach to workload.
