About the Role
We are looking for Youth Lead Practitioners to support adolescents and young adults aged 10–24 as they navigate life with a disability, from adolescence through to early adulthood.
Responsibilities
* Led the interdisciplinary team that plans a young person's NDIS funded therapy package, acting as the main contact for adolescent and young adult participants and their families
* Utilised assessment and intervention skills to youth participants to achieve their educational, personal, social, and working life goals
* Worked within the social model which means you'll have worked and delivered therapy in diverse settings, from homes and schools to playgrounds, workplaces and community centres
* Held meaningful, future-focused conversations with participants and their supports
* Empowered participants and their families to develop habits and routines
* Built meaningful relationships with your participants and their supports (family, carers, friends)
* Documented, and billed for, each session through our online participant management system
* Attended the weekly team meeting in the Hub, sharing stories of impact, coffee and laughs
* You'll have met with your Team Leader and Therapy Mentor to tap into their clinical knowledge and/or support
Qualifications and Experience
* Relevant tertiary qualification and registration.
* Bachelor or Master's in Youth Work or Community Services and YWA membership, if youth worker credentials.
* Bachelor or Master's in Social Work and AASW membership, if social worker credentials.
* Bachelor or Master's in Special Education and AASE membership, if specialist educator credentials.
* Bachelor of Nursing and AHPRA registration, if registered nurse credentials.
* Minimum 12 months experience working with youth aged 10-24
* A positive and flexible approach to work
* Strong communication skills and a team player
* Commitment to personal and professional growth
* Demonstrated experience working with people with a disability
* Willingness to work to billable targets
* A valid Australian driver's license
* NDIS Workers Screening Check
* Working with Children's Check
