The Opportunity:
We are seeking a dedicated Senior Support Worker to join our team. This role combines hands-on care with leadership and coordination to support participants in achieving their individual goals, while mentoring frontline staff and serving as the primary point of contact for onsite operations. You will work closely with participants, families, care teams, and external partners to ensure high-quality, person-cantered care aligned with each participant's care plan.
Key Responsibilities:
- Support participants in achieving their individual goals in line with their care plan, with a focus on independence, dignity, and well-being.
- Act as the onsite contact person for day-to-day questions from internal and external stakeholders, ensuring clear communication and timely problem-solving.
- Provide domestic and personal care services across both high and low needs, including assistance with medication as required and in accordance with policies and regulations.
- Mentor Support Workers and acting as a practice leader on site; ensure access to systems, documentation, and workload support.
- Report incidents, hazards, and complaints promptly and accurately; contribute to continuous improvement processes.
- Develop and maintain excellent relationships with key stakeholders, including participants, families, healthcare providers, local services, and partner agencies.
- Uphold professional boundaries, maintain confidentiality, and deliver compassionate, respectful, and culturally responsive care.
To be successful in this role you will have:
- Significant experience in the provision of personal care duties for participants with high and low needs.
- Strong commitment to quality customer service; ability to provide accurate and timely information.
- Demonstrated ability to relate to people from diverse backgrounds and treat everyone fairly.
- Clear understanding of professional boundaries in the execution of duties.
- Demonstrated ability to provide compassionate, respectful care and maintain participant dignity.
- Capability to mentor and support frontline staff; comfortable with documentation systems and workload management.
- Relevant certifications as required (e.g., First Aid, Medication Administration, Manual Handling, and any role-specific qualifications).
