About the Role
We are seeking an energetic and enthusiastic Clinical Care Coordinator to join our team on a full‐time basis!
You will be based at our Tarragal House, located at 107 Karalta Road Erina NSW 2250.
We're looking for enthusiastic candidates to assist with overseeing the delivery of high‐quality, person‐centred care. This role provides clinical oversight, supports care teams, and ensures compliance with aged care standards in line with Thompson Health Care's Philosophy of Care.
Qualifications
* Tertiary qualifications in Nursing
* Registered Nurse for a minimum of 3 years' experience with a mix of acute clinical and aged care experience (desirable).
* A good understanding of contemporary aged care, clinical practice and clinical leadership in a residential setting.
* Great communication skills and confidence to guide and actively contribute to a team.
* Strive for excellence in customer service and promote an environment of harmony and trust.
* Demonstrated high level organisational and time management skills.
Key Responsibilities
* Provide direct clinical care, including assessments and evaluations as required.
* Coordinate care with multidisciplinary teams and external providers.
* Support Registered Nurses and Care Staff in delivering high‐quality, person‐centred care.
* Ensure clinical care aligns with organisational policies, procedures, and care philosophy.
* Maintain accurate and compliant clinical documentation, including relevant legislation and ANAC requirements.
* Monitor, investigate, and escalate clinical concerns and incidents appropriately.
* Build positive relationships with residents and families to support individual care needs.
* Ensure compliance with various regulatory requirements including WHS, infection control, fire regulation and accreditation requirements.
