Job Description
We are seeking a Registered Nurse to join our multidisciplinary team in supporting individuals with complex mental health needs.
As a Registered Nurse, you will be responsible for delivering person-centered care and working alongside GPs, specialists, and other healthcare professionals. Your contributions will directly impact residents through comprehensive assessments, personalized care plans, medication management, education, referrals, and health promotion initiatives.
* Comprehensive assessments, including Mental State Examinations (MSEs) and risk evaluations.
* Developing and implementing personalized care plans.
* Administering and monitoring medications safely.
* Providing education and referrals to community services.
* Facilitating health promotion and social inclusion initiatives.
About the Service
Our Step-Up/Step-Down (SUSD) service provides contemporary, therapeutic mental health care through residential support for up to 28 days. We offer a range of supports through a multidisciplinary team to help people manage their mental health, avoid hospitalization, and live well in the community.
Our Step-Up services provide additional support for individuals managing a decline in their mental health, helping them avoid going to hospital when they become unwell. Our Step-Down services support individuals transitioning from hospital care, providing ongoing support to help them return home confidently.
Residents are partnered with a dedicated key worker who collaborates with them to develop a personal recovery plan. Groups, peer support, and individualized programs are designed to build new skills, confidence, and goals, while also providing referrals to ongoing support in the community.
Desired Skills and Qualifications
About You
We're looking for someone with:
* Tertiary qualification in Nursing and current AHPRA registration.
* Postgraduate qualification in Mental Health/addictions (or working towards one).
* National Police Clearance (≤6 months), First Aid/CPR certification, Working with Children Check, NDIS Worker Screening Check, and a valid Driver's Licence.
Additionally, we seek someone with:
* Experience in mental health or AOD settings, including clinical assessments, risk management, and care planning.
* Knowledge of psychiatric medications and medication management.
* Strong communication, teamwork, and problem-solving skills.
