As part of our multidisciplinary Carers Independent Living Support team, you'll deliver flexible, person-centred support across a range of programs including HACC and carer support services, alongside other community-based initiatives.
You'll work with people from diverse backgrounds, helping them navigate changing needs, stay connected, and build independence. This is a dynamic role that combines early intervention, practical support, and community engagement, all with a strong focus on inclusion and wellbeing.
What You Will Be Doing
* Building strong, trusting relationships with carers, clients, and families.
* Providing assessment, goal planning, and short-term targeted support.
* Coaching and connecting people with specialist services and community programs.
* Supporting group activities and community participation.
* Delivering practical assistance that promotes independence in daily life.
* Strengthening caring relationships and reducing social isolation.
* Working across multiple programs to provide coordinated, holistic support.

About You
A relevant qualification in Social Work, Community Services, Psychology, Counselling, or a related discipline, or demonstrated equivalent knowledge and experience relevant to community services practice.
