About this Role
You will play a key role in managing a caseload of clients, working closely with families, support teams, and health professionals to deliver integrated, high-quality care.
This is a full-time position, working Monday to Friday.
Main Responsibilities
* Conduct regular in-home visits to assess client needs and wellbeing
* Developing, implementing, and reviewing personalised care plans in consultation with clients and families
* Liaising with health professionals, social workers, and community services to ensure integrated care
* Managing budgets to ensure quality services are delivered
* Maintaining accurate records of assessments, care plans, and client interactions
