About the Opportunity
Help patients transition safely from hospital to home while making a meaningful difference in their healthcare journey. You will become part of a multicultural, social and close-knit team that values collaboration, compassion and patient‐centred care. This rewarding role focuses on supporting older patients through the discharge planning process, helping to ensure safe, timely and appropriate transitions of care. You will be supported by the Patient Flow Manager, fellow Discharge Planners and After Hours Managers, with access to a collaborative multidisciplinary team committed to achieving the best possible outcomes for patients.
What You Will Be Doing
In this role, you will coordinate and support the discharge planning process for patients aged over 65 years, ensuring their ongoing care needs are identified and addressed. Working closely with Nurse Unit Managers, nursing staff, medical teams, allied health professionals, patients, families and external care providers, you will facilitate effective discharge planning and continuity of care.
Your responsibilities will include:
* Supporting patients and families through the discharge planning process
* Developing patient‐centred discharge plans and care goals
* Coordinating meetings with patients, families and external providers, including residential aged care facilities and community services
* Liaising with multidisciplinary teams to support timely and appropriate discharge outcomes
* Maintaining accurate documentation and reporting
* Identifying barriers to discharge and assisting in developing solutions to support patient flow
About You
You are a compassionate and organised healthcare professional who is passionate about supporting older people and improving patient experiences.
To be successful in this role, you will:
* Have nursing experience and a strong understanding of patient care pathways
* Demonstrate excellent communication and interpersonal skills
* Be highly organised with strong documentation and coordination abilities
* Thrive in a collaborative multidisciplinary environment
* Be committed to delivering person‐centred care and achieving positive patient outcomes
A background in medical nursing or experience working with older patients will be highly regarded.
Current Authority to Practice as a Registered Nurse with AHPRA with relevant post‐registration qualifications and at least 3 years' experience working in Aged Care of their specified post‐graduated qualification or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.
