Job Title: Advanced Clinical Practitioner in Aged Care
The Murrumbidgee Local Health District is seeking an experienced Advanced Clinical Practitioner in Aged Care to join their team. This role involves providing expert clinical assessment, diagnosis, treatment, and management of older persons who present to the emergency department and may require continued care in the community.
Key Responsibilities:
* Providing high-level nursing skills in aged care and dementia, with extensive experience in nursing clients with complex medical, cognitive, and social needs/issues across various clinical settings
* Leading preventative health programs and patient education in various settings
* Utilizing evidence-based interventions to guide treatment and manage ongoing therapy for patients within scope of practice
* Collaborating with general practitioners and specialist medical officers to ensure comprehensive care for patients
Requirements:
* Current registration as a Registered Nurse with AHPRA, with endorsement as a Nurse Practitioner
* Minimum 5 years' experience working in aged care, with advanced nursing skills in this field
* Highly developed written and verbal communication skills, with proven interpersonal and negotiation skills
* Computer literacy with word processing and database programs
* Ability to work autonomously and within a multidisciplinary team
