About the role
This role operates within the Transitional Aged Care Program (TACP), which provides short-term, goal-oriented support to older people following a hospital stay. The primary aim is to help individuals regain independence and improve functional capacity through tailored care delivered in the patient's home. The position contributes to a multidisciplinary team focused on restoring health, mobility and confidence during the transition from hospital to community living. Join our compassionate team as an Occupational Therapist and make a real difference – help older adults stay active, independent and safe in the comfort of their own homes.
You will work closely with members of the TACP team, Allied Health Team Leader and TACP Manager. The role is a new position with scope for use of hybrid models of care delivery.
The Aged Care Division delivers a range of clinical and non-clinical services across the entire Local Health District to support older people in their families to enable them remain supported in their home. The aged care teams and programs have a strong commitment to advocacy, restorative care and reablement back to wellness and independence. The position will enjoy cooperation and support within a strong leadership team and can expect significant challenges in a changing aged care environment which needs to respond to a growing patient population and Commonwealth and state health reforms impacting on operations.
