We are seeking Carer Hub Navigators for the Toowoomba region. The role is full time, based in Toowoomba QLD. Stipend: SCHADS 4B ($43.08-$46.35 per hour) plus super and salary packaging benefits.
Responsibilities
* Act as the welcoming face and guiding hand for carers in the new Carer Hubs.
* Create seamless connections between carers and vital community resources.
* Build, maintain, and strengthen relationships with community organisations, health services, schools, aged care facilities and local government agencies.
* Provide consistent presence and reliable support to carers using a 'no wrong door' approach.
* Oversee the Carer Hub, facilitating social interaction and promoting wellbeing, connection and a sense of belonging.
* Integrate community development and engagement strategies to raise awareness and enhance accessibility to Carer Gateway services.
Qualifications
* Experience working with family carers in mental health, alcohol and other drugs, or community support services.
* Certificate IV or above in a support discipline and/or equivalent community sector experience.
* Current valid Driver's Licence.
* Strong technology skills and appropriate IT skills.
* Satisfactory National Police Records Check (within 12 months).
* WWCC / QLD Blue Card; NDIS Workers Screening check / QLD Yellow Card.
* Right to work within Australia.
* NDIS Worker Orientation Module Certificate.
