The Role
An opportunity has become available for an Allied Health Assistant (Transition Care Program) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
- Contribute to a multi-disciplinary team through departmental and team meetings, case conferences and other team projects and activities
- Provide a defined range of clinical screening assessments as delegated and allowed by testing guidelines and legislation
- Provide a defined range of treatments as prescribed by the allied health professional and work under their general direction
- Contribute to client records according to organisational guidelines and legal requirements
- Contribute to quality improvement activities with increasing ownership
About you
**To be successful in this position you will require**:
- A Certificate III in Aged Care or Certificate IV in Allied Health Assistance or equivalent would be well regarded
- Aged Care Check (QLD Health will meet the cost)
- Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella (Chicken Pox), Pertussis (Whooping Cough), Hepatitis B and COVID 19
- Tuberculosis screening, to identify if the tuberculosis vaccination may be required
- Driver Licence
- Declaration of serious discipline history must be completed

Salary
- Permanent full time
- Base salary ranging from $61 464 - $64 613 per annum

Transition Care Program (TCP) operates within the Allied Health and Rehabilitation service line in the Emergency and Specialty Services Directorate. TCP provides short term and active management for older people who have been hospitalised and who require more time and support in a non-hospital environment to complete their recovery. Based in Southport.
