About Central Gippsland Health
Central Gippsland Health (CGH) is a sub‑regional health service and the major provider of health and aged care services in the Wellington Shire. CGH is located in the City of Sale, 215 km east of Melbourne, and serves an immediate population of approximately 42,000 in Central Gippsland, reaching a wider community in East Gippsland for specialised services such as perinatal, critical care and surgery.
CGH provides acute care across three campuses (Sale, Maffra and Heyfield) and offers a full‑time emergency department, critical care units, neonatal critical care, operating theatres, day procedure unit, oncology, dialysis, general medical and sub‑acute rehabilitation services. It also delivers aged care and community services, including four residential aged care homes and a range of home‑based supports such as Support at Home and Commonwealth Home Support Programs. A multidisciplinary allied health team provides physiotherapy, dietetics, occupational therapy, speech pathology, social work and public dental services.
As a teaching hospital affiliated with Monash University's School of Rural Health, CGH provides placements for students and encourages Aboriginal and Torres Strait Islander peoples to apply.
About the Role
You will support the Maternal Child Health Service Team by improving the health and wellbeing of parents and children identified as being at risk of poor outcomes. Your work will include providing advocacy around social justice, human rights and social change, and delivering a comprehensive, focused approach for early intervention and prevention of physical, emotional and social issues. You will ensure families have a voice and facilitate timely referral for children and family support.
Duties
* Participate in intake and allocations, assessing client eligibility as per Enhanced Maternal and Child Health Program Guidelines.
* Conduct initial assessment, identify presenting concerns, gather family history (including a genogram) and establish a Child and Family Action Plan (CFAP).
* Apply specialist knowledge and skills to identify and address the needs of children and families, particularly children identified at risk of harm.
* Conduct risk assessments and report children at risk of harm to Child Protection in consultation with the Enhanced Maternal & Child Health Nurse and Program Leader.
Qualifications, Skills and Experience
* Demonstrated ability to work with families using strengths‑based and family‑centred principles.
* Recognition of differing social and cultural values and respect for client self‑determination.
* Extensive knowledge in early childhood development and the ability to recognise deviations from normal health and development.
* Ability to interact with a diverse range of clients in an empathetic, non‑judgmental manner, respecting rights, privacy and individuality.
* Ability to work constructively within a multidisciplinary service system and develop collaborative relationships with other service providers.
