Job Title: Care Manager
Care Managers at our organization are responsible for coordinating and overseeing personalized home care services. They ensure each client receives high-quality support based in their local area.
* Manage a caseload of clients, planning and reviewing goals to deliver person-centered care
* Communicate effectively with clients, families, and team members to develop and update care plans
* Meet with clients and families face-to-face to review care plans and ensure needs are being met
* Accurately document all client interactions and share relevant information with the team
About You:
* Previous experience working with seniors or aged care is essential
* A background in care coordination, case management, or client-facing support roles is required
* Ability to work under pressure and multitask shifting priorities is necessary
