* Partner with participants to design and implement personalised, strengths-based NDIS plans.
* Connect people with the services and supports that align with their goals and needs.
* Use recovery-oriented and person-centred approaches to build skills and confidence.
* Coordinate service delivery, monitor progress, and adapt plans as required.
* Collaborate with a multi-disciplinary team and contribute to service innovation.
* Maintain accurate records and ensure alignment with NDIS practice standards.

What You'll Bring:
* Significant experience in the disability or human services sector.
* Knowledge of intervention models and ability to coordinate services.
* Specialist knowledge of NDIS and related support services.
* Strong problem-solving and conflict-resolution skills.
* Ability to advocate effectively for participants and promote their independence.
Your Toolkit:
* Tertiary qualification in community services, social work or equivalent and extensive community work and training (essential)
* Current DHS Working with Children Check (essential)
* Current SA driver's licence (essential)
* Current Safe Environments for Children and Young People – Through Their Eyes Certificate (essential)
* Support Coordination experience (preferred)
