About the Role
We are seeking a passionate and community-focused IndigenousOutreach Worker to join our Integrated Team Care / Allied Health Services teambased in Emerald.
This role works closely with clients, allied healthprofessionals, and community stakeholders to support Aboriginal and TorresStrait Islander people living with chronic health conditions.
The successful candidate will help facilitate access tomultidisciplinary care, promote health awareness, and strengthen relationshipsbetween mainstream primary care and Indigenous health services.
Key responsibilities include:
* SupportingAboriginal and Torres Strait Islander clients to access appropriate healthand wellbeing services
* Promotingthe Closing the Gap and Integrated Team Care (ITC) programs
* Buildingstrong relationships with GPs, health professionals, and communityorganisations
* Assistingclients to better manage chronic health conditions
* Deliveringculturally appropriate, non-clinical support services
* Advocatingfor improved health outcomes within the community

About You
You are passionate about improving health outcomes forAboriginal and Torres Strait Islander peoples and have the ability to buildstrong, respectful relationships across community and health sectors.
To be successful in this role, you will have:
* Relevantqualifications and/or demonstrated experience in a similar role
* Highlydeveloped interpersonal and relationship-building abilities
* Understandingof the health challenges impacting Aboriginal and Torres Strait Islandercommunities
* Experiencedelivering culturally safe and sensitive support services
* CurrentQLD C Class Driver Licence
* CurrentCPR and First Aid Certificate
* CurrentBlue Card (or willingness to obtain prior to commencement)
* SatisfactoryPolice Check
* Abilityto meet vaccination requirements including:
* Measles,Mumps, Rubella
* HepatitisA & B
* Pertussis(Whooping Cough)
