Excel Community Care is seeking a full-time Community Worker to support individuals, families and vulnerable community members through the provision of welfare support, referrals, advocacy, care coordination and community engagement services. The role involves assessing client needs, implementing support programs, monitoring client wellbeing and liaising with community agencies and service providers.

Key Duties and Responsibilities
* Assess clients' needs and assist in developing appropriate support plans and welfare strategies.
* Conduct client interviews and monitor client progress through ongoing support and reporting.
* Provide information, referrals and assistance to clients requiring welfare and community support services.
* Liaise with community organizations, healthcare providers and welfare agencies to coordinate services for clients.
* Monitor and report on the progress of clients.
* Liaise with community groups, welfare agencies, government bodies and private businesses about community issues and promote awareness of community resources and services.
* Refer clients to agencies that can provide additional help.
* Interview clients and assess the nature and extent of difficulties.
* Support individuals and families experiencing social, emotional and financial difficulties.
* Assist in the implementation of community support programs and client engagement activities.
* Maintain accurate client case notes, reports and service records.
* Promote awareness of available community resources and support services.
* Participate in multidisciplinary collaboration to ensure holistic support for clients.
* Ensure compliance with organisational policies, confidentiality obligations and community care standards.
Position Requirements
* Relevant qualification in community development, development studies, welfare, social services or a related field.
* Relevant experience in social support, welfare services, community care or client support environments.
* Strong communication and interpersonal skills.
* Ability to work with culturally diverse and vulnerable clients.
* Demonstrated organisational and case management abilities.
* Minimum 2-3 years of community work experience is required.
* Diploma-level qualification is required for this position.
* Valid Australian work visa or work authorisation is required.
