Job Description
* Deliver end-to-end case management services, including intake, needs assessments, and service delivery plans.
* Assess and prepare individual risk assessments, safety planning, and implement strategies to achieve goals.
* Engage clients in decision-making processes regarding their case management plans.
Required Skills and Qualifications
* Hold relevant qualifications in Community Services, Case Management, Counselling, or Social Work.
* Possess extensive work experience in case management or a similar role.
* Hold a current Open C Class Queensland driver's license.
* Holder of a Working with Children Blue Card or the ability to obtain one.
* Willingness to undertake a National Police Clearance check.
