Summary:
We are seeking a highly organized and compassionate Case Manager to join our team. The Case Manager will be responsible for managing and coordinating the care of clients, ensuring that they receive the appropriate services and resources to meet their needs. The ideal candidate will have excellent communication skills, be able to work independently and as part of a team, and have a robust commitment to client advocacy.
Responsibilities:
- Conduct assessments of clients' needs and develop individualized care plans
- Coordinate and monitor the delivery of services and resources to clients
- Advocate for clients and ensure that their rights are protected
- Maintain accurate and up-to-date client records
- Collaborate with other professionals, such as healthcare providers and social workers, to ensure comprehensive care for clients
- Provide education and support to clients and their families
- Participate in case conferences and team meetings
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Qualifications:
- Bachelor's degree in social work, psychology, or a related field
- Minimum of 2 years of experience in case management or a related field
- Knowledge of community resources and services
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Ability to maintain confidentiality and handle sensitive information
- Proficiency in Microsoft Office and electronic medical records systems
