Summary:
We are seeking a highly organised and detail-oriented Support Coordinator to join our team. The Support Coordinator will be responsible for providing administrative support to ensure productive operation of the organisation. The ideal candidate will have excellent communication skills, be able to multitask and prioritise work effectively, and have a strong attention to detail.
Responsibilities:
- Service Coordination: Liaise with participants, their families, and service providers to coordinate and manage a range of supports that align with the participant’s NDIS plan.
- Planning and Implementation: Assist participants in understanding and implementing their NDIS plans, helping them to identify and connect with appropriate services, such as health, housing, education, and community participation.
- Budget Management: Support participants in managing their NDIS budgets, ensuring they make informed decisions about their funding and support options.
- Advocacy: Advocate on behalf of participants to ensure their rights and preferences are respected and that they receive high-quality services that meet their individual needs.
- Risk Management: Identify potential risks or barriers to the participant’s progress and work proactively to mitigate these challenges.
- Reporting and Documentation: Maintain accurate and up-to-date records of participant interactions, progress, and outcomes, ensuring compliance with NDIS requirements and SCHADS Award standards. Reporting to the NDIA.
- Teamwork and Communication: Collaborate with service providers, stakeholders, and NDIS to provide seamless support to participants. Be able to communicate effectively with clients, their families and external service providers.
- Health and Safety: Ensure a safe working environment by adhering to all relevant health and safety regulations, policies and procedures.
- Compliance: All support coordinators – registered and non-registered, are required to follow the NDIS Code of Conduct. Registered support coordinators must also meet the quality standards set out in the NDIS Practice Standards and Quality Indicators and ensure that workers in risk assessed roles have an NDIS Worker Screening Check.
Qualifications:
- Tertiary qualification in Social Work, Disability, Community Services, or a related field.
- Experience in the disability sector and a strong understanding of the NDIS framework.
- Minimum of 2 years’ experience in a similar support coordination role
- Excellent communication, negotiation, and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organisational and time-management skills.
- A commitment to upholding the principles of choice, control, and empowerment for people with disabilities.
