This is a full time position, fixed term for 12 months and can be located in Queanbeyan or Tuggeranong.
About The Role
The Care Partner plays a central role in supporting older people receiving Support at Home services to live well, independently and safely at home. The role is responsible for building trusted relationships with older people, families and carers, understanding what matters most to each person, and co‐designing support plans that reflect individual goals, preferences and needs.

Duties
* Coordinate and review care services, monitor budgets and delivery
* Respond to changing client needs and adjust support plans
* Maintain accurate records to support quality, person-centred care
* Communicate effectively and apply sound judgement to build strong support relationships
Skills & Experience
* Relevant tertiary qualification in aged care, community services, nursing, social work, allied health or a related field, or equivalent experience.
* Demonstrated experience as a Care Partner, Case Manager, Care Coordinator or in a similar community aged care role.
* Sound understanding of community aged care, including Home Care Packages, Support at Home, aged care reforms and person-centred care approaches.
* Strong communication and interpersonal skills, including the ability to build genuine relationships with older people, families, carers and service providers.
