Senior Clinical Care Coordinator

We are seeking an experienced Senior Clinical Care Coordinator to join our team. In this role, you will be responsible for maintaining a positive and supportive work environment while mentoring and coaching staff to foster excellence in care delivery.

The successful candidate will provide clinical leadership and coordination across assigned homes, ensuring person-centred care principles are adhered to. This includes conducting regular assessments of resident care to monitor progress, identify potential risks, and implement interventions.

You will coach, mentor, and support nursing and care staff to enhance their clinical knowledge, skills, and performance. We require someone with strong leadership and mentoring abilities who can adapt to different team dynamics and clinical needs.

Key Responsibilities:

* Maintain a positive and supportive work environment while mentoring and coaching staff.
* Provide clinical leadership and coordination across assigned homes.
* Conduct regular assessments of resident care to monitor progress and identify potential risks.
* Coach, mentor, and support nursing and care staff to enhance their clinical knowledge, skills, and performance.


Requirements:

* Clinical Expertise: Senior clinical care experience.
* Strong leadership and mentoring abilities.
* Excellent written and verbal communication skills.
* Flexibility to work across multiple homes adapting to different team dynamics and clinical needs.
* Registered Nurse (RN) with current AHPRA registration.
* Knowledge of relevant aged care regulations, standards, and guidelines.
