About Sunraysia Community Health Services
Sunraysia Community Health Service (SCHS) is a dynamic organisation promoting health, wellbeing and independence in a social model of health. The organisation is a source of primary and preventive care for our community members who are at risk of poor health outcomes, and experience acute health disparities. SCHS understands the influence of the Social Determinants of Health upon health inequities experienced by community members and work with consumers and community members, to structure their care around the context in which they live and work.
The Palliative Care Program (PCP) provides proactive, client-centred care to individuals with advanced chronic illnesses and life-limiting conditions. Referrals to the program should be made as early as possible following diagnosis.
Palliative Care:
* Affirms life and recognises dying as a normal process
* Enhances quality of life and may positively influence the course of illness
* Neither hastens nor postpones death
* Advocates for relief from pain and other distressing symptoms
* Integrates psychological and spiritual aspects into client care
* Supports clients to live as actively as possible until death
* Offers support to families and carers during the client's illness and throughout bereavement
* Uses a multidisciplinary team approach to meet client and family needs, including bereavement counselling where appropriate
* Is applicable early in the illness trajectory, alongside curative or life-prolonging therapies.
Key Selection Criteria
* Current registration with AHPRA as a Registered Nurse.
* Current, unsuspended and unrestricted drivers licence.
* Demonstrated understanding of the term "Palliative Care".
* Highly developed, demonstrated skills and recent experience in providing Palliative Care to clients and carers.
* Highly developed interpersonal and communication skills.
* Demonstrated experience communicating effectively with persons from diverse backgrounds and cultures.
* Understanding of how the issue of privacy and confidentiality relate to client care.
* Ability to work across a seven day week roster.
* Computer skills, particularly the Microsoft Office Suite.
