About the Role
SA Health's Northern Adelaide Local Health Network is seeking a dedicated Social Worker AHP2 to join our Rehabilitation in the Home (RITH) team at Modbury Hospital. This part‐time, long‐term contract position runs until 31 December 2026, offering stability and career growth in Adelaide's northern suburbs.
Key Responsibilities
* Conduct comprehensive psychosocial assessments for hospital patients
* Develop and implement discharge plans with patients and families
* Collaborate with healthcare teams to ensure seamless care transitions
* Provide crisis intervention and brief counselling services
* Connect patients with community resources and support services
The Social Worker must work towards and meet the State‐wide Generic Core Competencies Framework.
Qualifications
* Appropriate Degree or equivalent qualification granting eligibility for full membership of the Australian Association of Social Workers
