It's not often that truly rewarding Coordinator roles arise. At Pivot Support Services we have an opportunity for the right person to deliver case coordination in an existing community service, grow it and make it even better. You will have to be seeking a challenging and rewarding role where you can leverage your experience and make a real impact in the lives of people living with disadvantage and vulnerability.

This is not a role that anyone can do. You'll have to be tough and resilient and thrive on challenges of a multi-functional role, providing direct client support, case management, community engagement and program delivery. We want someone who can deliver high quality, person-centred supports across three key areas: Reintegration, NDIS Support Coordination and the Community Hub. You will need to promote dignity, wellbeing, inclusion and empowerment for people transitioning from custody, people living with disability and community members experiencing disadvantage. The role requires strong professional integrity, accurate record keeping, collaboration with internal and external stakeholders and the ability to work in a dynamic environment with diverse client needs.

Job Description

Job title: Coordinator

Reports to: Co-Managers

Responsible for (other positions): N/A

Type of position: Full time, Part-time may be considered

Location: Albany. May be required to travel to other locations for periods to perform duties.

Agreement/ Grade: Social, Community, Home Care and Disability Services Industry Award 2010 (Social and Community Services Employees) Level 3.1 - 3.4 plus 5% above award plus access to car for home and community visits, gifted leave, salary sacrifice benefits.


Introduction

The primary purpose of the position is to provide direct supports to participants in NDIS and/or Reintegration and/or Community Hub program areas.


Key Responsibilities


Client Support & Case Management

* Provide person-centred, culturally respectful case management across Reintegration, NDIS and/or Community Hub programs.
* Conduct assessments, develop individual support or reintegration plans, and review progress regularly.
* Maintain professional boundaries, confidentiality, and culturally safe practice.
* Complete intake, triage, risk assessments, and referral processes as required.
* Support clients to access services, build capacity, and work towards their goals.
* Provide crisis support, emergency relief (Community Hub), and structured reintegration planning (Reintegration).


Program Delivery & Stakeholder Engagement

* Facilitate programs, workshops, and client activities within prisons, community settings, and the Hub.
* Coordinate referrals and maintain collaborative relationships with government agencies, service providers, justice partners, and community organisations.
* Contribute to program planning and continuous improvement initiatives.
* Represent Pivot professionally at stakeholder meetings as delegated.


Reintegration Responsibilities (if assigned)

* Manage a caseload of pre- and post-release clients.
* Deliver strengths-based reintegration planning and goal setting.
* Facilitate programs, mentoring, and group activities in custodial and community environments.
* Comply with prison access protocols and correctional services requirements.


NDIS Support Coordination Responsibilities (if assigned)

* Maintain a Support Coordination Level 2 and/or Specialist Support Coordination Level 3 caseload.
* Assist participants to understand and implement their NDIS plans.
* Coordinate supports, monitor provider delivery, and build participant capacity.
* Ensure compliance with NDIS Practice Standards and reporting obligations.


Community Hub Responsibilities (if assigned)

* Provide client support, case management, and referrals for individuals experiencing hardship.
* Coordinate emergency relief, community engagement activities, and wellbeing programs.
* Draft and support funding applications to ensure continuity of services.


Administrative, Compliance & Reporting

* Maintain accurate, timely case notes and client documentation in line with regulatory and organisational requirements.
* Support program reporting, data entry, and evidence collection for audits and evaluations.
* Adhere to privacy, safeguarding, and confidentiality obligations.
* Follow standard operating procedures (SOPs) consistently across all tasks.


Workplace Health and Safety

* Follow all WHS requirements and promote safe work practices.
* Participate in hazard identification, incident reporting, and risk management.
* Take reasonable care for personal safety and the wellbeing of clients and colleagues.


Other Duties

* Maintain mandatory clearances (WWCC, NDIS Worker Screening, DOJ).
* Undertake additional duties as directed to support organisational objectives.
* Assist other program areas during peak demand or staff absence.
