Employment Term: Permanent Full-Time
Location: Cairns and surrounding locations
Remuneration: $42.15 - $59.04 per hour + Super + 5 weeks annual leave + Salary Packaging + Penalties & Allowances
Relocation allowance: Up to $5,000 for relocation costs.

Your role in our team
A Social Worker supports clients living independently in the community by providing psychosocial assessment, care coordination, advocacy, counselling, and services that improve quality of life and well‐being.
Key Responsibilities
* Conduct comprehensive psychosocial assessments and risk management for community clients.
* Provide counselling, emotional support, crisis intervention, and advocacy.
* Develop and review individualised care plans.
* Work collaboratively with multidisciplinary teams including personal care staff, nursing and allied health.
* Facilitate referrals to community supports, health services and government programs.
* Promote client choice, dignity and rights‐based practice.
Qualifications
* Tertiary qualification in Social Work.
* Registration with Australian Association of Social Workers.
* Experience in aged care or acute settings (desirable).
* Excellent interpersonal and written communication skills.
* Experience with technology (smartphones, tablets, laptops).
* Current Australian driver's licence and road‐worthy registered vehicle.
