About Us
NBMLHD is an organization delivering a comprehensive range of public health services across the Nepean, Blue Mountains, and Lithgow Regions.
The Primary Care and Community Health Unit provides services for children, families, adults and carers across community venues, homes and telehealth.
Our Supportive and Palliative Care Service works with hospital and community teams to ensure holistic, respectful, evidence-based care at every stage of the care journey.
We are seeking an experienced and passionate Clinical Nurse Consultant (CNC) to join our multidisciplinary Supportive and Palliative Care team.
This position is based across Lawson, Lithgow and Hawkesbury Community Health Centres.
What You Will Bring To The Role
Current Registered Nurse Division 1 with AHPRA, with 5 years full-time equivalent postgraduate experience and a minimum of 3 years full-time equivalent experience in Supportive and Palliative Care nursing.
Postgraduate qualifications relevant to the specialty or equivalent experience as deemed appropriate by the organisation.
Proven clinical leadership skills and ability to work collaboratively within a multidisciplinary framework, with commitment to nursing excellence.
Strong communication and interpersonal skills across various mediums; understanding of the practice development framework.
Proven ability to implement changes and guide clinical practice using evidence-based research.
Experience in strategic and clinical service planning, including change management and cultural change processes.
Clinical teaching skills, competency with contemporary information systems, and development of specialty education programs.
Clinical expertise in client-centered consultancy with the ability to work independently.
Current valid NSW drivers licence with a good driving record and willingness to travel long distances across the Local Health District.
Employment of a temporary visa holder may occur only if no suitable permanent resident or citizen of Australia has been identified and must not exceed visa conditions.
Responsibilities
Provide expert clinical leadership, consultation and education to enhance palliative care delivery across community and inpatient settings.
Work with patients, carers and health professionals to optimise symptom management, quality of life and end-of-life care aligned with individual goals and values.
Demonstrate compassionate, proactive, person-centred care and strong leadership, communication and teamwork.
