About the opportunity
As a Program Worker, you will work towards community member recovery by developing and monitoring individual recovery plans with participants, setting skill and knowledge development goals, and providing direct support to achieve social inclusion.
Your role also includes encouraging participants to use wellways services in pursuit of their goals, assisting with exit planning from the programme and helping build long‐term support networks.
Responsibilities
* Develop, monitor and update individual recovery plans with participants.
* Provide direct support to participants to achieve social inclusion and individual goals.
* Encourage participant utilisation of wellways services to advance their recovery.
* Assist participants in planning programme exit and building long‐term support networks.
* Record activities and complete regular reports in Microsoft Office Suite.
Qualifications
* Relevant tertiary qualification or demonstrated experience in a social science or related discipline.
* Prior experience working in the Mental Health industry or within a community‐based organisation.
* Effective verbal and written communication skills, including report preparation.
* Proficiency in Microsoft Office Suite.
* Current valid Driver's Licence with ability to travel for the role.
* Current Work with Children Check.
* Right to work in Australia.
* Satisfactory National Police Records Check.
