Bayside Health is a public health service delivering high‐quality care across metropolitan Melbourne and surrounding regions. This casual position is based at Caulfield Hospital.

The Role

The Community Development Worker (CDW) collaborates with carers and the people they care for to address issues and needs. They provide assessment, advocacy, and consultation, determine suitable pathways for service provision, and work with carer support groups, networks and service providers across community, health and hospital settings. The CDW supports vulnerable and diverse groups to strengthen accessibility of services for carers.

Key Responsibilities
* Work with carers and care recipients to address issues and needs and facilitate solutions.
* Provide assessment and advocacy, and/or consultation with carers and the people they care for.
* Determine suitable pathways for service provision through Carer Gateway, Support for Carers Program and Commonwealth Home Support Program.
* Collaborate with carer support groups, networks and service providers across community, health and hospital settings.
* Support and strengthen accessibility of services for carers, especially vulnerable and diverse groups.
Qualifications and Experience Required
* Tertiary qualifications in Health, Behavioural Sciences, Social Sciences, Community Development or Welfare.
* Minimum 3 years' experience and knowledge of at least two practice areas (Aged, Dementia, Disability, Mental Health, Youth Services).
* Demonstrated understanding of issues facing carers and people from diverse communities and available services.
* Experience in community development, promotion, event development and working with a broad service system.
* Experience in casework and care coordination, including needs/risk assessment and support planning.

Additional Information

In accordance with the Health Services Amendment (Mandatory Vaccination of Healthcare Workers) Act 2020, health care workers in Category A or B roles are required to be vaccinated against influenza or hold an acceptable medical exemption.
