About The Role

Join a dedicated and collaborative team delivering meaningful support to patients with complex care needs during critical points in their healthcare journey. As the Complex Care Social Worker, you will provide targeted interventions for patients requiring additional support to access safe, coordinated care in community settings, helping to improve continuity of care and patient outcomes.

Working as part of a dynamic multidisciplinary team within the Emergency Department, you will play an important role in assessment, intervention, care coordination, and discharge planning to support safe, timely transitions from hospital to home or community-based care. You'll work closely with Community Services, including the Planned Care for Better Health team, to strengthen engagement pathways and develop practical, patient‐centred strategies for ongoing support.

This role offers a highly collaborative environment, partnering closely with ED Social Workers, the JHH NDIS team, and a range of community‐based services to deliver holistic and integrated care for patients with complex needs.

You'll also have the opportunity to broaden your experience through potential rotations across the wider JHH and Belmont Social Work teams and participation in the Social Work On Call roster, providing variety, professional development, and exposure to diverse clinical settings.

Requirements

* Social Work degree and eligibility for the Australian Association of Social Workers (AASW)
* Demonstrated advanced clinical experience (minimum 3 years) in crisis intervention including trauma, counselling, case management for patients/families with illness adjustment, domestic violence, child protection, end of life care and bereavement support
* Demonstrated understanding of, and experience with, guidelines and policies relevant to ED Social Work practice, including child protection protocols, NSW Health policies, End of Life Care Guidelines, and initiatives such as Closing the Gap and Cultural Respect.
* Capacity to participate in the After Hours On‐Call and weekend backup roster, providing covering for crisis presentations to JHH/JHCH between 10pm and 8am

Additional Information

* To be eligible for employment in this position you must be an Australian Citizen, or permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport, or hold an appropriate visa that allows you to live and work in Australia. Employment of a temporary visa holder will only be offered in line with the conditions and expiry date of your visa.
* An eligibility list will be created for future temporary part time vacancies.

This is a Category A position.
