About the Role The position of Senior Complex Care Coordinator will: Lead the development of service capability to implement sustainable recovery-oriented service systems for the identification, treatment, and support of infants, children and young people with complex care needs, incl.
those with a disability (physical, intellectual and developmental) who have co-occurring mental health needs, and their families, Facilitate responsive care for infants, children and young people (and their families) whose mental illness is compounded by complex care needs, Establish and maintain strong cross-sectoral links with local and statewide stakeholders to inform, and be informed by, current service provision for infants, children and young people (and their families) with complex care needs; and Improve collaboration between stakeholders to enhance the timely delivery of an integrated, recovery-oriented service response to help achieve the best possible outcomes for these consumers and their families.
