About the Role
You will be supporting clients and families within aged care settings across community and residential services.
Responsibilities may include:
* Conducting assessments with clients and families
* Facilitating SAT, EPA and EPG applications
* Completing financial applications
* Assisting with accommodation applications and aged care placements
* Liaising with families, multidisciplinary teams and external providers
* Supporting clients through aged care transition processes
What We Need From You
* Bachelor of Social Work
* Relevant Social Work qualification and eligibility for membership
* Experience within aged care social work and/or NDIS settings
* Strong understanding of aged care processes within WA
* Experience conducting client assessments
* Excellent communication and interpersonal skills
* Ability to work independently within fast‐paced environments
