Staying On Facilitator
This role plays a vital part in supporting young people as they transition from Out-Of-Home Care to independent living.
Key Responsibilities:
* Develop Staying On Agreements for continued living arrangements until the young person turns 21.
* Authorise and manage Staying On subsidies, resolving related issues with the Department of Communities.
* Provide therapeutic support and guidance to carers, including parenting strategies and conflict resolution.
* Coordinate review and support of Staying On Agreements with Transition Coaches, young people, and carers.
* Access safe and stable housing for young people, including allowances and community housing options.
* Facilitate case conferences and family intervention meetings as needed.
* Support family mapping, reunification, and contact with biological families.
* Maintain accurate case management records and contribute to internal reporting.
* Build relationships with stakeholders, including the Department of Communities, carers, kin, and community members.
* Participate in interagency meetings and professional development activities.
