Full-time, permanent role in Queanbeyan, NSW. Salary $39.57 per hour plus superannuation and not‐for‐profit salary packaging.

Responsibilities
* Provide person‐centred, recovery‐focused case management that supports participants to build independence, wellbeing and community connection.
* Develop, implement and regularly review individualised, strengths‐based support plans in line with participant goals.
* Deliver outreach and community‐based support, including assistance with daily living skills and practical needs.
* Conduct intake, assessment and transition planning to support entry into and exit from the program.
* Build strong, respectful relationships with participants, promoting choice, dignity and self‐determination.
* Collaborate with internal teams and external services to coordinate holistic and effective supports.
* Complete accurate case notes, reports and data entry in line with organisational, funding and compliance requirements.
Qualifications
* A related degree or diploma or minimum Certificate IV in Mental Health or Peer Work.
* Current valid WWCC and driver's licence.
* Demonstrated understanding of recovery focused, person‐centred practice and psychosocial support.
* Experience providing case management and practical support to people with complex needs.
* Strong interpersonal and communication skills with the ability to build trust and rapport.
* Ability to work collaboratively with colleagues, partner organisations and community services.
* Sound organisational skills, including timely documentation, reporting and use of client management systems.
* A professional, values‐driven approach with the ability to manage complexity, ambiguity and boundaries.
