About the role
We are looking for a dedicated Registered Nurse, Occupational Therapist, Physiotherapist, or Social Worker to support older Australians. The role involves delivering aged care assessment services to enable older adults to remain living independently at home or access government‐funded aged care services.
Key responsibilities
* Conduct home support and comprehensive aged care needs assessments for individuals aged 65 and older.
* Develop and review support plans to assist clients in achieving their goals and living independently.
* Collaborate with a multidisciplinary team to evaluate clients' physical, psychological, and social needs and determine eligibility for government‐funded aged care services.
* Document identified needs and service recommendations for the older adult using tools, templates and the My Aged Care Portal.
* Conduct culturally sensitive assessments for groups such as Aboriginal and Torres Strait Islander individuals, those from CALD backgrounds, the LGBTIQ+ community, or the homeless.
* Promote healthy ageing and well‐being using a "wellness and reablement approach."
* Be a vital part of a team dedicated to enhancing the quality of life and independence of older adults.
About you
The ideal candidate is a diligent, empathetic, and driven individual who is motivated by a passion to assist vulnerable members of the community and who possesses the following:
* A tertiary qualification in Nursing, Social Work, Occupational Therapy, or a related health field with registration with relevant professional bodies (e.g., AHPRA).
* Proven experience as a Clinical Assessor in an aged care setting (preferred), with at least one year of demonstrated experience delivering services within aged care environments and/or supporting older persons.
* Flu vaccinations, current First Aid and CPR certification (or willingness to obtain).
* Experience conducting holistic, client‐centred assessments.
* Excellent interpersonal and communication skills, both written and verbal.
* A genuine passion for supporting the independence and well‐being of older adults, strong problem‐solving skills, and the ability to manage competing priorities effectively.
* Proven ability to use technology, including laptops and tablets, to efficiently conduct and manage client assessments.
* Willingness and ability to travel within the region, including between client locations and to other remote areas as required with prior notice.
* Willingness to complete training requirements specified under the My Aged Care Workforce Learning Strategy 2023.
