Job Description
Entry level position as a community / disability support worker, providing supports to individuals and assisting in group activities. Work is casual with full time hours possible.
JOB SUMMARY
The Community Support Worker plays a crucial role in delivering high-quality, person‐centred support to individuals with disabilities. This position involves working closely with participants to assist them in achieving personal goals, enhancing their quality of life, and promoting independence. The ideal candidate will be compassionate, patient, and committed to making a positive impact in the lives of others.
RESPONSIBILITIES Personal Care
* Assist participants with daily living activities, including bathing, dressing, grooming, and toileting, ensuring their dignity and comfort.
Health and Wellbeing
* Monitor and support participants' health needs, including administering medication, attending medical appointments, and promoting healthy lifestyle choices.
* Report health and safety incidents as required through the Employment Hero application.
Social Support
* Facilitate social interactions and community participation by accompanying participants to social events, recreational activities, and community programs.
Skill Development
* Support participants in developing life skills such as cooking, budgeting, and using public transportation, fostering their independence.
Employment Support
* Support participants to find and keep employment, through skill development and workforce participation strategies outlined in an agreed plan.
Transport
* Provide participant transport as required under agreed plans.
Documentation
* Maintain accurate and up-to-date records of participant progress, incidents, and any changes in their needs or circumstances in the Nightingale application.
