This role will drive the design, delivery, and embedding of our Neighbourhood Health Hub (NHH) located in Hornsby NSW. We are looking for an experienced Implementation Lead who can turn a complex model into a real, functioning service across multiple partners and systems, leading implementation from readiness through to evaluation in a sustainable, community‐led manner.
Responsibilities
* Lead end‐to‐end implementation of the NHH model across all five stages
* Apply implementation science frameworks (CFIR, ERIC, Proctor outcomes, ABCD) to guide delivery
* Coordinate the staged rollout of Small Teams, Team of Teams, and Hub Facility model
* Build strong cross‐sector partnerships with health, NGO, council, and community stakeholders
* Embed co‐design with people with lived experience and priority populations
* Use local data, asset mapping, and evaluation frameworks (RE‐AIM, Proctor) to drive decisions
* Support commissioning alignment, sustainability planning, and capability building across providers
* Contribute to tools, frameworks, and reporting that support system‐wide implementation
Qualifications
* Relevant tertiary qualifications in health, public health, community services, psychology, social work, implementation science, project management or a related discipline, or equivalent demonstrated experience
* Experience supporting implementation, service development, project coordination, evaluation support or practice improvement within health, community services or a related sector
* Experience in implementation and supporting multi‐site projects or community‐based implementation
* Experience in supporting co‐design, consultation and developing practical resources, training materials and guides
* Demonstrated experience and ability to gather, synthesize qualitative learnings from projects, stakeholder engagement and service delivery activities
* Strong written communication skills, including experience preparing reports and summaries
* Demonstrated ability to work collaboratively with diverse stakeholders across health, community, research, government and not-for-profit settings
* Strong organisational and project management skills with the ability to effectively manage competing priorities across multiple projects
