Flexible hybrid GP role combining in-clinic care in Ipswich with remote multidisciplinary case conferencing. $275/hr (in-clinic) | $220/hr (remote) | Veteran Health
Responsibilities
* Deliver preventative and chronic care consultations to Veterans
* Areas include: joint health, diabetes education, skin checks, chronic pain management, and mental health referrals
* Participate in multidisciplinary case conferencing with Social Workers and Allied Health (Physio, Exercise Physiology, Podiatry, Dieticians, etc.)
* Conduct care reviews and continuity checks to ensure coordinated, comprehensive care
Offer
* Hybrid schedule – 1 day in-clinic weekly (Ipswich Clinic) + 1 day remote weekly (Telehealth/case conferencing) (Thursday preferred)
* Competitive pay – $275/hr (clinic) and $220/hr (remote), inclusive of GST

Qualifications
* Vocationally Registered GP with current AHPRA registration
* Confident in both face-to-face and Telehealth consultations
* Reliable, collaborative, and proactive in team-based care
* Interest and/or experience in Veteran-focused healthcare
