Care Management Lead

St Louis Home Care is seeking an experienced Care Management Lead to join our Home Care leadership team and play a key role in delivering high‐quality, person‐centred Support at Home services.

This senior position supports older people to live safely, independently and with dignity in their own homes by overseeing client onboarding, care management and customer service functions across Adelaide and the Fleurieu Peninsula.

St Louis Care is made up of four specialist service areas:

* St Louis Homes – NDIS-approved provider for younger onset dementia
* St Louis Home Care – Government-approved and private home care provider
* St Louis Nursing Home – Boutique residential aged care
* St Louis Allied Health – FIFO allied health services supporting remote South Australian communities
About the Role:

Reporting to the Home Care Manager, and working closely with the Clinical Services Lead and Home Support Services Lead, the Care Management Lead is responsible for:

* Leading timely and effective client onboarding, assessments, care planning and reviews
* Overseeing care management and complex case coordination, including multidisciplinary case conferencing
* Ensuring transparent and accountable individual budget management aligned to client goals and funding
* Providing leadership, coaching and supervision to geographically dispersed care management teamsDriving consistent, high‐quality customer service and person‐centred practice
* Supporting compliance with the Aged Care Act, Strengthened Quality Standards and Support at Home requirements
* Using data, reports and KPIs to inform continuous improvement and service planning
About You:

You are a confident and capable leader with strong experience in community aged care or NDIS, and a passion for quality, governance and developing high‐performing teams.

You will bring:

* Demonstrated leadership experience supporting large, geographically dispersed teams. Minimum of 5 years
* Strong understanding of Support at Home care management, funding, budgets and service agreements
* High‐level communication, engagement and negotiation skills with clients, families, carers and stakeholders
* Sound judgement and the ability to manage risk, safeguarding and escalation appropriatelyCommitment to culturally safe, inclusive and trauma‐aware practice
* Strong digital literacy, documentation oversight and data‐informed decision making
Essential Qualifications and Clearances:
* Certificate IV in Community Services (or higher)
* National Police Clearance
* Overseas criminal convictions statutory declaration (if applicable)
* Full and current Australian Driver's Licence
* Ongoing commitment to annual flu vaccination
