We are seeking a dedicated Care Advocate to join our team.
About the Role
The Care Advocate plays a vital role in coordinating and managing consumer care packages, ensuring that all services are delivered in a coordinated, person-centred, and cost-effective manner. This involves providing exceptional support to consumers eligible for Support at Home services funded by the Commonwealth Government, as well as Care coordination for consumers eligible for the HACC PYP Linkages Program and Support Coordination for participants eligible for the National Disability Insurance Scheme (NDIS).
Key Responsibilities:
* Consumer Support: Provide high-quality care partner services to consumers eligible for Support at Home services, working closely with them to understand their needs and preferences.
* Care Coordination: Coordinate care packages for consumers eligible for the HACC PYP Linkages Program, ensuring seamless delivery of services.
* Support Coordination: Facilitate Support Coordination for participants eligible for the National Disability Insurance Scheme (NDIS), empowering them to take control of their lives.
