Job Title: Care Coordinator
The Team Leader plays a pivotal role in delivering high-quality care to residents. The key responsibilities include coordinating the care needs of residents across the facility, ensuring holistic and person-centered care provision. This involves supporting the Clinical Care Coordinator and Registered Nurse to supervise and coordinate care.
Key Responsibilities:
* Provide social and emotional support to all residents
* Support the coordination of the care needs of residents across the facility
* Lead/supervise care on the shift, within their designated wing or area
* Perform medicine-related tasks in accordance with scope of qualification, state or territory legislation, regulation, and policies
Requirements:
* Diploma of Enrolled Nursing (with APRHA Registration to administer medication) or Certificate IV in Ageing Support (including HLTHPS007 Administer & Monitor Medicines)
* Certificate III in Individual Support with willingness to undertake training to obtain required qualifications
* High level of commitment, integrity, honesty, and trustworthiness
* Effective listener with ability to communicate at all levels both orally and in writing
* Ability to understand and use information technology and programs
