Compensation is SCHADS Level 4 – $***** – $***** per hour, plus super and salary packaging benefits.

Qualifications
* Previous experience in mental health or case‐management outreach support (professional, volunteer, or lived experience), supporting people with complex needs such as ABI, AOD, mental illness, physical disability or dual diagnosis.
* Tertiary studies in Mental Health, Social Science or a relevant qualification (or actively studying) are required.
* A requirement to meet all compliance requirements in relation to NDIS Work Screening, Working With Children's Check, work rights, and to possess a current and valid driver's licence.
About the Role
As part of the TAC team, you will support participants on WorkCover and TAC throughout their recovery journeys. Participants may have a mental health condition, acquired or traumatic brain injury, or a disability resulting from a workplace or traffic incident. Your role is to help them build the skills and confidence to manage daily tasks independently, emphasising empowerment over direct intervention.
The position operates in a hybrid model, combining work‐from‐home, an office in Dandenong South, and outreach support. The program covers a large catchment area in Dandenong and the South‐East suburbs.
Responsibilities
* Community outreach and telehealth support.
* Case management and implementation of established behavioural support plans.
* Regular communication with care teams and linking participants to a wide range of community and clinical services.
* Developing professional relationships with individuals, their families and other stakeholders in the community.
* Managing referrals, conducting risk assessments, and providing accurate, timely case notes and reports.
* Use of a variety of systems and navigation across multiple platforms.
These positions are permanent full‐time, Monday to Friday, 9am‐5pm.
