Physiotherapist – Home and Community Care

- Location: Southern Central Coast (Gosford, Kincumber and surrounds)
- Employment type: Permanent Part Time or Full Time
- Pay rate: up to $54.50 per hour (based on experience) + up to $18,550 tax free + superannuation
- Sponsorship: available for candidates with AHPRA seeking PR

Day‑to‑day responsibilities
- Conduct comprehensive assessments of clients' physical and functional abilities.
- Develop and deliver individualized or group treatment plans based on clients’ needs, goals and preferences.
- Implement evidence‑based physiotherapy interventions to address mobility issues, pain management and other age‑related conditions.
- Regularly assess and report clients’ physical and psychosocial capabilities to the care management team.
- Maintain, prepare and support precise records, exercise sheets, assessment reports and progress notes that reflect clients’ physical function.
- Make recommendations regarding exercise programmes, mobility equipment, exercise equipment, home safety and refer to other disciplines as required.
- Uphold ethical and professional standards in accordance with regulatory guidelines.
- Collaborate with interdisciplinary teams, including care advisors, occupational therapists, fitness instructors, allied health assistants and other healthcare professionals, to provide holistic care.
- Educate clients and families on preventive measures and exercises to improve mobility.
- Participate in team meetings, case conferences and training sessions to contribute to continuous improvement of care delivery.
- Engage with team members, clients and families in a positive and respectful manner, welcoming a culturally diverse team.
- Build relationships with clients and their families, understanding the challenges they face in the evolving aged‑care landscape.
Required qualifications
- Bachelor or postgraduate degree in Physiotherapy and current AHPRA registration.
- Excellent interpersonal, verbal and written communication skills.
- Ability to manage all aspects of compliance.
- Demonstrated experience with multidisciplinary team working.
- Effective time‑management skills to ensure patient intervention, reports and outcomes are achieved timely and ability to prioritise and organise own caseload.
- Computer literacy.
- Knowledge and experience of health, aged and/or disability services.
- Current driver’s licence and registered and insured motor vehicle.
