Job Title: Team Leader Disability Services
CareChoice is a registered disability services provider that delivers a wide range of services and provides complex support across the Disability sector in various settings, including home care, residential, community and Specialist Disability Accommodation.

Key Responsibilities:
* Supervising, managing, and motivating team members to ensure quality services and safeguards to clients that promote opportunities for meaningful participation and active inclusion in society.
* Coaching, mentoring, guidance, and support to identify risk-increasing behaviors.
* Maintaining and auditing client records for compliance purposes.
* Crisis management and incident report follow-up.
* Assessing staff performance on an ongoing basis.
Requirements:
As a positive role model with a 'can-do attitude', you'll have a high degree of initiative and strong desire to achieve outstanding results. It's not essential, but if you have a nursing/social work/psychology background, it would be advantageous.
Ideally, you'll have:
* Strong experience as a Team Leader in the Disability or similar space.
* Previous experience supporting clients with complex needs.
* Relevant qualifications in the disability and community services (Certificate IV in Disability or equivalent).
* A genuine desire to ensure that clients receive exceptional service.
* Willingness to work as part of a team and cultivate a strength-based approach.
