Job Summary

This is a challenging and rewarding role for a skilled professional who wants to make a positive impact in the lives of marginalized and vulnerable adults. As an Outreach Practitioner, you will be part of a multidisciplinary team that provides flexible, accessible, and non-stigmatizing services to individuals who find it difficult to engage with mainstream health and social care services.

The successful candidate will have excellent communication and advocacy skills, as well as the ability to work independently and as part of a team. You will be responsible for delivering outreach services to hard-to-reach groups, assessing and managing client cases, and developing and implementing individualized care plans.

Key Responsibilities

* Deliver outreach services to marginalized and vulnerable adults, including asylum seekers and refugees, black and minority ethnic groups, gypsies and travellers, and migrant workers.

* Assess and manage client cases, develop and implement individualized care plans, and provide ongoing support and monitoring.

* Work collaboratively with other professionals, including healthcare providers, social workers, and community organizations, to ensure comprehensive and coordinated care.

* Contribute to the development and implementation of policies and procedures related to outreach services and client care.

Requirements

">
">
* Degree in Occupational Therapy, Social Work, or Nursing, or equivalent experience and qualifications.
">
* Strong communication and advocacy skills, with the ability to work effectively with diverse populations.
">
* Ability to work independently and as part of a team, with excellent problem-solving and organizational skills.
">
* Experience working with marginalized and vulnerable populations, preferably in a community-based setting.

Working Conditions

This role involves working in a variety of settings, including community centers, hospitals, and clients' homes. The successful candidate must be able to travel extensively, using public transportation or personal vehicle, and work irregular hours, including evenings and weekends.
