Case Manager – Home Care Packages | Darling Downs Region

Are you a passionate and experienced Case Manager looking to make a meaningful impact in the lives of older Australians? We're working with a well-established aged care provider who is seeking a dedicated professional to support their Home Care Package consumers across the Darling Downs region.
In this role, you'll be delivering person-centred case coordination, supporting clients to live independently at home with dignity and purpose. You'll play a key role in intake assessments, ongoing care planning, and holistic case management, all within a collaborative, wellness-focused framework.
Key Responsibilities:
* Develop strong, trusting relationships with clients and their families/carers to create tailored, effective care plans
* Ensure packages are fully utilised, waitlists are maintained, and budgets are managed efficiently
* Deliver case management aligned with best practice standards, current legislation, and organisational policies
* Where qualified, provide discipline-specific interventions as part of your scope of practice
* Support the transition to the new Support at Home Program commencing 1 July 2025
We're Looking For:
* Previous experience in Aged Care as a Case Manager, Care Coordinator, or similar
* Confidence working autonomously and managing competing priorities
* Excellent communication skills with a compassionate, client-first approach
* A relevant qualification (Diploma level highly regarded)
* Strong understanding of Home Care Packages and consumer-directed care
* Current Police Clearance, First Aid & CPR Certificate
* Open Driver's Licence, own vehicle, and willingness to travel within Toowoomba and occasionally to Brisbane
