Community Nurse Role
We are seeking a Registered Nurse to provide clinical care in the home for older Australians.

You will be responsible for delivering wound care, catheter and stoma management, diabetes support and chronic disease management. You will complete comprehensive health assessments and develop personalized care plans in collaboration with multidisciplinary teams including GPs, carers and allied health professionals.
To be successful you must have a Division One RN registration and at least 1 year of experience in community, aged care or acute settings. Proficient use of technology is also essential.
