POSITION DETAILS
The Program Coordinator leads and manages the delivery of Home Support and Centre Based Services to ensure high quality consumer driven care aligned with contractual requirements. The role oversees day to day operations case management and coordination of supports with a focus on wellness reablement dignity and independence to help clients live safely at home. This position also provides operational supervision to staff volunteers and the Activities Coordinator while ensuring services are flexible responsive and tailored to meet the diverse needs goals and circumstances of clients and carers.
RESPONSIBILITIES Program Management
* Lead and coordinate aged care services to ensure effective delivery aligned with funding requirements legislation and quality standards
* Support program growth and continuous improvement including implementation of aged care reforms and wellness reablement and restorative care approaches
* Oversee client service delivery including care planning complaint resolution and ensuring outcomes meet individual needs and goals
* Build and maintain strong stakeholder partnerships to support referrals service delivery and community engagement
* Ensure accurate documentation reporting data management and compliance with program guidelines and contractual requirements
* Manage projects and contribute to program evaluation reporting and ongoing service development
People Management
* Provide leadership supervision and support to staff volunteers and the Activities Coordinator to promote high quality client centered care
* Build team capability through coaching supervision performance reviews and workforce planning
* Foster a collaborative team culture focused on best practice client outcomes and continuous improvement
* Support recruitment and workforce planning in partnership with management and HR
* Ensure compliance with organisational policies workplace health and safety and professional standards
* Participate in supervision learning and development and contribute to a positive organisational culture
KEY SELECTION CRITERIA
* Relevant bachelor degree in social work social science nursing or related discipline with significant experience in community aged care in a senior role
* Strong understanding of aged care services including wellness reablement restorative practice and strengths based case management
* Knowledge of aged care legislation reforms and consumer directed care principles
* Demonstrated leadership experience with the ability to support develop and manage team performance
* Experience working with diverse client groups including older people people with mental illness dementia disability and their carers
* Strong stakeholder engagement data analysis communication and organisational skills with ability to manage budgets reporting and competing priorities
* Must have a current driver licence and willing to obtain a NDIS Worker's Screening Clearance
* Current First Aid & CPR Certificate
